Patient Satisfaction Surveys

As a hospital patient you may receive two separate patient satisfaction surveys from us in the mail. Both are important and we hope you will take the time to complete and return them.

The blue survey comes back to the hospital and provides us with immediate feedback about the care and service that you received while a patient here. In addition, a random selection of patients will receive a patient satisfaction survey as part of a national initiative by the United States Department of Health and Human Services.
